How do you best titrate Allopurinol?

Allopurinol is usually the first choice to lower uric acid...but prescribers often don't give a high enough dose.
Suggest starting with allopurinol 100 mg/day and titrating to a serum uric acid less than 6 mg/dL. Explain that it's okay to go up to 800 mg/day in patients with good renal function. |

Is Uloric better for renal insufficiency?

Reps will say that Uloric (febuxostat) doses don't need to be adjusted for renal impairment. But keep in mind that allopurinol doesn't either...until creatinine clearance is 20 mL/min or less.
Reps will also say that Uloric lowers uric acid more than allopurinol...but this is compared to just 300 mg/day of allopurinol. |
