Phase 1: Describe the problem
Different rates of health care coverage, average
annual income lower than other groups, different
lifestyle factors. May be reluctant or uninformed
about getting regular screenings (cancer, cholesterol,
blood sugar, blood pressure, etc.)
Phase 2: Analyze Problem
The intervention included establishing policies in the
community environment to support increased
consumption of fruits and vegetables. Communication
components included church bulletins, brochures,
recipe booklets, posters, banners, educational materials, and sermons with a health promotion component
Phase 3: Plan Intervention
The planning team
drafted a timetable to determine their budgetary needs
and they proposed that the nutrition intervention project
would last for four Years.
Phase 4: Develop Intervention
The planning team identified
the relevant stakeholders to a meeting to discuss the program
intervention and the need for evaluating it during and after
the intervention occurred.
Phase 5: Plan Evaluation
Once the planning team approved the proposed communication and evaluation plans, they merged the two plans into one integrated plan. This merging process required that the entire planning team meet a number of times to ensure the activities would be carried out in a coordinated fashion.
