What are the 3 laboratory findings in primary folate deficiency?
1)Increased homocysteine
2)Increased volume in RBC's
3)Decreased RBC folate content
What is the treatment for primary folate deficiency?
Folic acid or Leucovorin (N5-formyl-THF)
What are the physical findings of someone with cobalamin deficiency?
Decreased peripheral sensation
What are the 5 laboratory findings of someone with cobalamin deficiency?
1) Increased Homocysteine
2) Increased methylmalonic acid and propionic acid in the blood
3) Increased volume of RBC's
4) Decreased concentration of cobalamin in the blood
5) Anemia
What is the Folate Trap Hypothesis?
What are the two components?
Insufficient cobalamin leads to decreased THF and increased N5-methyl-THF.
1) Decreased N5-methyl-THF + Homocysteine --> THF
2) Decreased SAM leads to decreased feedback inhibition of N5/N10-methylene-THF
What is the treatment of cobalamin deficiency?
IM injection of Vitamin B12 every 1-2 months or oral Vitamin B12.
Why is it important to test for cobalamin deficiency when a folate deficiency is found.
Cobalamin deficiency and folate deficiency both lead to megaloblastic anemia. Cobalamin deficiency can lead to irreversible brain damage.
What foods provide folate?
Grains, fruits, legumes, green leafy vegetables.
