what are early decels ?
Occur only during contractions as the fetal head is compressed (as seen during pushing)

are mirror images of the contractions
not dangerous to fetus
what are late decels?
Begin well after the contraction begins (often near the peak)

Reflect impaired placental exchange or Uteroplacental insufficiency (can lead to fetal compromise)

Require nursing intervention to improve placental blood flow and fetal oxygen supply

indicates fetal hypoxia
what are variable decels?
Caused by reduced flow through umbilical cord (cord compression)
what are the non-reassuring patterns?
 Tachycardia
 Bradycardia
 Decreased or absent variability
 Late decelerations
 Variable decelerations (falling to less than 70 beats per minute for longer than
60 seconds)
 Prolonged decelerations
 Hypertonic uterine activity
what is the POA for non-reassuring patterns of ANY kind?
1. Stop Oxytocin (Pitocin) infusion
2. Reposition woman to avoid the supine position and/or improve pattern
explain amnio infusion?
Addition of sterile fluid into the uterus to supplement amniotic fluid

It helps to cushion the fetus ad the cord. Also used to flush out Meconium fluid.
what is the purpose of a cord blood gas?
done after delivery to check the pH. Co2, O2 and HCO3
baseline fetal heart rate? FHR
average rate 110-160 measured over 2 mins
bradycardia
less than _110 bpm for at least_10_min.
tachycardia
more than _100_bpm for at least _10_min.
some causes of brady cardia are
◦ Acute hypoxia
◦ Drugs
◦ Arrhythmia’s
◦ Heart block
◦ Hypothermia
what are some causes of tachy cardia?
◦ Maternal fever
◦ Fetal infection
◦ Hypoxia
◦ Drugs
◦ Hyperthyroid
◦ Arrhythmia’s
what is variability in regards to FH monitoring?
Fluctuations in baseline FHR: the normal irregularity of the FHR

The MOST important indicator of fetal well being;Reflects neurological modulation of the FHR

This is what causes the printed line to have an irregular, wavelike appearance rather than a smooth, flat one.
what are the four types of variability?
 Absent- Flat Line
 Minimal- A little change
 Moderate (Average) - Normal
 Marked- Big changes
