What am I?
Sinusitis (rhinosinusitis)
symptoms:

sudden onset, strep exposure, headache, no cough, vomiting (in younger children)

MC in children 4-7

what am I?
bacterial pharyngitis
symptoms:
-hoarseness, dysphonia (in addition to other URI symptoms), odynophonia, dysphagia, odynophagia, dyspnea, rhinorrhea, postnasal discharge, sore throat, congestion, fatigue, malaise

Physical exam:
-erythema and edema of the vocal folds, increase in secretions, irregularities of the surface contour of the vocal folds.
Laryngitis
Symptoms: usually abrupt onset
-low grade fevers up to 104, sore throat, myalgias, headaches, progressive dry cough
-less common are rhinitis, weakness, fatigue, diarrhea (more common in children), encephalopathy

physical exam:
-febrile (100-104 F), tachycardia, pharyngitis, red, watery eyes, dry cough
Influenza
symptoms:
-abrupt onset of fever, sore throat, respiratory difficulties, drooling, dysphagia, distress, anxiety

Physical exam:
-pt appears seriously ill or toxic, “hot potato” voice, stridor, sniffing position, cough is rare, lymphadenopathy may be present, swollen erythematous epiglottis
epiglottitis
symptoms:
-rhinorrhea, congestion, obstruction of nasal breathing
-sore/scratchy throat, odynophagia, dysphagia
-non productive cough
-Halitosis, hyposmia, headache, sinus symptoms, photophobia/conjunctivitis (MC adenoveirus), fever (usually low grade, but can be high), GI symptoms, myalgias, fatigue/malaise

physical exam – pts often have lack of major clinical findings despite subjective symptoms, nasal mucosal erythema and edema, nasal discharge, halitosis, low grade fever

What am I?
nasopharyngitis
Symptoms
-Nasal symptoms – precede other sxs by 1-2 days
-Rhinorrhea (usually clear), sneezing, pruritus, and congestion
-May be accompanied by facial pain/pressure, altered sense of smell, and postnasal drainage
-Also sore throat, eyes burning/tearing
-Dry non productive cough, malaise
-Usually coexists with other URIs

Physical exam:

Throat: erythema, palatal petechiae, exudate, tonsillar hypertrophy, vesicular oropharyngeal lesions, foul breath

Lymphadenopathy, murmur, hepatosplenomegaly, rash

pharyngitis
Symptoms:
-starts w/ slow onset of nasal irritation, congestion, and coryza. Progresses over 12-48 hrs to fever, hoarseness, cough, and stridor. Barking cough that is worse at night. Hoarseness in older children and adults. Symptoms persist for 3-7 days w/ a gradual return to normal

physical exam:
-usually don’t appear toxic, stridor, wheezing, retractions, posturing, cyanosis, lethargy and agitation may represent hypoxemia.
