Mild - Comedonal

*1st line = Topical retinoid


*Alternatives = SA, azelaic acid, glycolic acid

Mild - Papular/Pustular

*1st line = topical retinoid +/- topical antimicrobial


*Alt = same as comedonal (SA, Azelaic acid, glycolic acid)

Moderate - Papular/Pustular

*1st line = Oral antibiotic + topical retinoid +/- BPO


*Alt = 1st line +/- azelaic acid


*Alt for females = oral hormonal therapy + 1st line alts

Moderate - Nodular

*1st line = Oral antibiotic + topic. retinoid + BPO


*Alt = Oral isotretinoin or 1st line plus azelaic instead of BPO


*Female alt = Oral hormonal therapy +/- everything pretty much

Severe - Nodular/Conglobate

*1st = Oral isotretinoin


*Alt = High dose oral antibiotic + tropical retinoid + BPO

*reduces inflamed lesions and lesion count


*more affective from lotions and hydroalcoholic gels

Tazarotene

*Short contact therapy (1-5 min)


*Good for sensitive skin


*Preg. Category X

Tretinoin

*Reduces number of microcomedones and comedones


*Category C but not to be used while pregnant

Lesion count

*Decrease by 10-25% within 4-8 wks or more than a 50% decrease in 2-4 months


*After that see primary care

Comedones

*Resolve by 3-4 months


*If not see primary

Inflammatory lesions

*Resolve within a few weeks
