Why is it important to observe and re-observe your patient?
In order to determine his condition and to detect any changes.
When is the ONLY time you should skip the ongoing assessment?
When you're too busy providing life-saving interventions.
Define "ongoing assessment."
A procedure for detecting changes in a patient's condition. It involves four steps:

1) Repeating initial assessment

2) Reassessing and recording vital signs

3) Repeating focused assessment

4) Assessing adequacy of interventions
How can the mental status of an unresponsive child be checked?
Shouting or flicking of feet
When checking interventions, what should you do?
1) Try to take a fresh look at the patient

2) Ensure adequacy of oxygen delivery and artificial ventilation

3) Ensure management of breathing

4) Ensure adequacy of other interventions
Define "trending."
Changes in a patient's condition over time, such as slowing respirations or rising pulse rate, that may show improvement or deterioration, and that can be shown by documenting repeated assessments.
What must you do immediately after obtaining a vital sign?
Record it.
When repeating a focused assessment, what changes might you see frequently in patients?
1) Stomach distention appearing or increasing

2) Paradoxical motion becoming apparent
What should you do with your oxygen delivery equipment from time to time during transport?
Check the entire path from tank to patient -- check regulator, flowmeter, tubing, mask, bag, etc.
What do you do if the nonrebreather bag completely deflates when the patient inhales?
Increase flow rate.
How can you reassess the condition of bleeding wounds?
1) Check bandages

2) Palpate injuries on the back, or that are tough to see, so you can check your gloves for blood
What miscellaneous interventions must you be sure to re-check from time to time?
Cervical collars, backboard straps, splints.
How often do you reassess a stable patient? And an unstable patient?
Every 15 minutes if stable, every 5 if unstable
