-either open, bilateral chest tubes or close Baptize with Betadine Left arm up to patient's head Cut under breast/pec curving into the armpit starting at the anterior axillary line (skin, SC tissue, fat) Big scissors (Mayo) through intercostals and cut up to sternum and down to bed Rib spreader with crossbar to the bed open 8-10 inches (ribs will break) 1 - Release tamponade (push the lung up, pickups and small scissors to nick the anterior part of the pericardium), if blood cut pericardium vertically and express the heart, extend clamshell incision, examine heart, plug the hole with your finger OR staple OR saline foley 2 - Control hemorrhage -LUNG - crush lung until you can see the inferior pulmonary ligament, cut it, pull out lung and twist it -INTERCOSTALS - pressure -R chest tube or hand over to confirm no blood in R chest 3 - CPR - open chest CPR with 2 hands shelling, aortic cross-clamp (30m) with hand tracking chest wall to 1st tube |
