Furosemide (Lasix)

Class
Loop Diuretic: Affects the distal (ascending) loop of Henle.
Route
Slow IV push (20 mg /min)
Innitial Dose
40 mg

If pt. is already taking furosemide at home, administer twice the pt's usual single daily dose up to a maximum of 100 mg.

Pediatric Dose: not used in prehospital setting
Drug Action
-Peripheral vasodilator: Decreases peripheral vascular resistance

Decreases cardiac workload and oxygen demand on the heart.

Increases water excretion by inhibiting reabsorbtion of water, sodium and chloride in the kidneys.
Indications
Pulmonary edema due to CHF or kidney failure.

NOTE: common problems occure in EMS when a pt. is incorrectly diagnosed in the field as having "pulmonary edema" but actually has pneumonia.
Furosemide (Lasix)

Precautions
Must be preceded by NTG

Monitor ECG and blood pressure closely

Document any urine output
Side Effects
-Hypotension

-Arrhythmias secondary to electrolyte imbalances

-Deafness, if given too rapidly
Contraindications
Systolic BP less than 110

Pregnancy

Dehydration
