Classification
Adrenergic agonist, inotrope, vasopresser
MOA
Stimulaters alpha and beta adrenergic recepters. At Moderate doses dopamine stimulates beta 1 recepters resulting in inotropy and increased cardiac output while maintaining dopamingeric induced vasodilatory effects.
At high doses alpha adrenergic agonism predominates and increased peripheral vascular resistance and vasoconstriction result.
Indications
Hypotension and decreased cardiac output associated with cardiogenic shock and septic shock, hypotension after return of spontaneous circulation flowing cardiac arrest, symptomatic bradycardia and unresponsibe to atropine.
Contraindications
Pheochromocytoma, VF,VT, or other ventricular arrhythmias known sensitivity, Correct any hypovolemia with volume fluid replacement before adminsteresting dopamine
Side Effects
Tachycardia, arrhythmias, skin necrosis, sever hypertension from excessive vasoconstriction, angina, dyspnea, headache and vomiting
Cautions
Half life 2 minutes

Dosage /Route of administration
Adult: 2-20mcg/kg/min to desired effect. Cardiac dose is usally 5 to 10 mcg/kg/min. Vasopressor dose is usally 10 to 20 mcg/kg/min. Little benefit is gauned beyond 20 mcg/kg/min.
