Cardiac Glycoside, Antidysrhythmic, Inotropic Agent
Digoxin (Lanoxin)

MOA
1. Positive inotropic effect
2. Slows AV conduction
3. Increases cardiac output
4. Decreases chronotropy and dromotropy
5. Inhibits sodium/potassium pump, thereby increasing intracellular calcium levels

Indications
1. Atrial Fibrillation/Atrial Flutter
2. Refractory SVT
3. CHF

Contraindications
1. Ventricular Fibrillation
2. Ventricular Tachycardia (unless due to CHF)
3. Patient has already taken normal therapeutic dose
4. 2nd and 3rd degree heart blocks

Side Effects
Cardiovascular: bradycardia, AV blocks, dysrhythmias
Neurological: confusion

Precautions
1. AMI and renal failure
2. Need to decrease joules by 50% during cardioversion
3. Pregnant or nursing woman
4. Infants and children

Interactions
1. May increase risk of dysrhythmias
2. Use with Beta Blockers may decrease actions of Digoxin
