1. Begin the Process: when patient going to die in 6mo-1yr by physician who knows them best
2. Appropriate degree of truthfulness
3. Patient centered issues: hopes and dreams, funeral, location of death, medical decisions (power of attorney, living will) etc
4. Medical Treatment Issues: decide how decisions will be made, values history, clarify goals of care, how will actively treat symptoms
5. Resuscitation Preferences: CPR (little value for people who's heart stops because they are dying, more value for people who aren't dying but their heart stops), artificial hydration, antibiotics, etc
