What is oxygen therapy used for?
Used to ensure an acceptable SaO2 (usually 89% is OK)
What are problems with oxygen therapy?
Expense
Supports combustion Tethering CO2 retention: Loss of hypoxic drive to breathe Increased CO2 causes sedation |
How can CO2 retention with oxygen therapy be avoided?
Titrate oxygen slowly, monitoring mental status and SaO2.
When can aerosol therapy be administered?
Quiet breathing
BiPAP Mechanical ventilation |
What is the purpose of airway clearance measures? Who does not need it?
To prevent atelectasis
A patient who can cough productively |
2 functions of mechanical ventilation
Reduce work of breathing
Improve gas exchange |
MOA of mechanical ventilation?
Change airway pressure - increased pressure results in increased flow - overcoming resistance and elastance
How does negative pressure ventilation work?
Apply negative pressure to the body and hence the lungs in a sealed box. Outside air will move in from higher atmospheric pressure to lower intrathoracic pressure (seldom used for modern care).
What are the 2 principal modes of ventilation?
Pressure cycled
Volume cycled |
Describe pressure cycled ventilation.
Pressure is applied to the airway for a given time. No tidal volume is guaranteed.
Describe volume cycled ventilation.
Volume is delivered using whatever pressure is required.
