Benign intracranial HTN:
Fat woman with headache, morning vomiting, & papilledema with a normal neurological examination.

CSF will show high pressure but all other aspects are normal

Looks like bacterial meningitis CSF(high protin, low glucose) but the cells are predominantly lymphocytes.

Tx
RMSF(Rickettsiae): fever headache and tired then rash on extremities that ends up on trunk.

Tx: Doxycycline or chloramphenicol

Lyme(Borrelia): Erythema migrans then migratory arthritis and bells palsy(b/l or u/l)

Kids<9: Amoxicillin
>9 to adult: Doxycycline

-Granulomas are non-caseating
-Can have optic neuritis & Erythema Nodosum(necrotic ulcer on the shin)
-Can have HS-megaly
-Can have rash all over trunk & back

X-ray: B/l hilar lymphadenopathy w/ b/l interstitial infiltrate which will progress to fibrosis

Malabsorption(steathorrhea=bulky smelly stools that float w/ weight loss) & herpetiformis dermatitis(itchy knee & elbow rash that has IgA deposits on biopsy).

-Have Anti-Gliadin Ab(IgA)
-Have atrophic villi w/ lymphocytic infiltration

Dx: biopsy Small bowel
Tx: No Gluten diet
When to screen for cancer in:
Prostate
Colon
PSA at 40

Colonoscopy & Guiac at 50

low dopamine at substantia nigra and excess Ach causes:

Shuffling gait,mask face,pill-rolling tremor, cogwheel rigidity.

Tx: Anticholinergics or

Amantidine/Bromocryptine
