aspirate the joint and send to pathology (gold standard)

Monosodium urate crystals are needle shaped and display a negative bifringence under a polarizing light microscope
For gout how do you tell if someone is an overproducer or underexreter?
TAke a 24 hour urinalysis
Which is more common to be - an undrxcreter or an overproducer?
Underexreters make up about 90%
What different medicines would you give a pt with gout depending on whether he was an underexcretor or overproducer?
overproducer --> allopurinol

For gout --> histo shows glucophage engulphing crystal (looks like martini and olive)
What do you send specimen in if you suspect gout?
alchoho, because formaldehyde dissolves gouty tophi.

If you're really not sure if it's gout you could send 2 specimens, one in normal formaldehyde and one in gout, but I'm not sure if there's any point to that.

PO 8 mg, IV 4mg
