-Frequency of seizure activity
-type and date of last seizure
-medications
-triggers of trends of seizure
-oxygen, oral airway, section equipment at bed side
-padded side rails
-educate family and caregivers not to place anything in cx. mouth in the event of a seizure

During a Seizure
-Cx. should not be restrained
-Lower cx to the bed or floor
-Protect cx head
-Romove nearby furniture
-Provide privacy
-Put cx on side if possible
-Loosen clothes
-Note length of time of seizure
-Note movement during seizure

After a Seizure
-Airway is clear
-Educate cx on what happen
-Provide comfort, understanding, recovery space
-Document
-Report seizure to provider
