Most studies are too small to produce precise estimates of disease frequency and measures of association and effect. How would you reduce random error on estimates on effect?
Why is it important to have a comparison group
Comparison group is hardly ever disease free. E.g. non drinkers also have road traffic injuries, not just drinkers. Traffic injuries among drinkers, not necessarily caused by alcohol. (Can be caused by another confounding factor).
