REcords are the folders in each master file. Each folder represents one thing stored in the master file. Example: Each patient has a record in the Patient Master File |

A contact is a date-specific snapshot of the data within an individual record. EX: all infomration about a patient that applies to a recent encounter is stored in a contact. |

A discrete piece of information about a record. EX: when you register a patient, each filed you fill out is an item (marital status = item, english fluency = item). The item is the question prompt. |
