Therapeutic applications of Alpha blockade
-Essential Hypertension - lowers blood pressure by causing vasodilation
-Reversal of Toxicity from alpha1 agonists
-Benign Prostatic hyperplasia- reduced contraction of smooth muscle in the bladder reduces the urge to void constantly
-Pheochromocytoma- suppress hypertension
-Raynaud's disease-can prevent vasoconstriction that makes you feel cold
Adverse effects of Alpha blockade
-Orthostatic hypotension
-Reflex Tachycardia- trying to restore normal blood pressure
-Nasal congestion- dilate the blood vessels in the nose and produces nasal congestion
-Inhibition of Ejaculation due to alpha1 receptor block
Therapeutic use of Prazosin
- treat hypertension
-Not approved but could be used for BPH (prostate enlargement)
Adverse effects of Prazosin
-orthostatic hypotension
-reflex tachycardia
-inhibition of ejaculation
-nasal congestion
What should patients be educated about with adverse effects of alpha blockade
S/S of orthostatic hypotension-dizziness, lightheadedness, and should be advised to sit or lie down if these occur
--also tell them that it can be minimized by avoiding abrupt movements
Other Alpha blockers
Dozazosin
Tamsulosin
Alfuzosin
Silodosin
Phentolamine
Therapeutic Applications of Beta Blockade
-Angina Pectoris- decrease cardiac work
-Hypertension-?? not really used for this anymore
-Cardiac Dysrhythmias-decrease the rate of sinus nodal discharge and suppress conduction of atrial impulses through the AV node
-Myocardial infarction
-reduction of perioperative mortality
-Heart failure- standard care
-Hyperthyroidism
-Mirgaine Prophylaxis
-Stage fright
-Pheochromocytoma
-Glaucoma
Adverse effects of Beta1 Blockade
-Bradycardia
-Reduced Cardiac Output
-Precipitation of Heart failure
-AV heart block
-Rebound Cardiac excitation
Adverse effects of Beta2 Blockade
-Bronchoconstriction
-Hypoglycemia from inhibition of glycogenolysis
Propanolol therapeutic uses
-Hypertension
-Angina Pectoris
-Cardiac dysrhythmias
-myocardial infarction

-prevention of migraine headache
-stage fright
Adverse effects of Propranolol
-Bradycardia
-AV heart block
-Heart failure
-Rebound cardiac excitation
-Bronchoconstriction
-Inhibition of glycogenolysis
-CNS effects- depression, insomnia, nightmares
-Effects in neonates
Contraindications for propranolol use
- Severe allergies (it works the opposite of epinephrine which is used to treat anyphlactic shock)
-Diabetes
-Cardiac, Respiratory and Psychiatric Disorders- can be exacerbated by propanolo
Drug interactions of propanolol
-Calcium channel blockers- can cause excessive cardiosuppression (increases effects)
-Insulin
Metoproplol Therapeutic Uses
-Hypertension (Main)
-angina pectoris
-heart failure
-myocardial infarction
Adverse effects of metoproplol
-bradycardia
-reduced cardiac output
-av heart block
-rebound cardiac excitation following abrupt withdrawal
