What are the three different types of hormones regarding solubility?
- Lipid soluble hormones that are bound to protein carriers and are temporarily water soluble and bind to receptors inside the cell
-Unbound lipid soluble hormones that are free in plasma and can bind to receptors
- Water soluble hormones are free in plasma and need no carriers and bind to receptors on the cell surface
What is the most common cause of SIADH (posterior pituitary dysfunction)?
What are other causes?
- Most common is small cell CA in the lung
- Other causes are CNS disorders (trauma, CVA, brain surgery), drugs (anesthesia, narcotics), stressors (pain, fear, surgery, trauma)
What are the clinical manifestations of SIADH?
What are the treatments?
- Manifestations include: Hyponatremia (causes swelling of the tissues, esp. the brain), hypo-osmolarity, decreased urine output, concentrated urine
- Treatments include: Fluid restrictions, monitor daily weight, monitor I&O, monitor serum Na+, monitor neurological manifestations through the Glascow Coma Scale
What are the manifestations of Hyponatremia?
- Manifestations include swelling of the tissues, esp. of the brain, which present through anorexia, vomiting, fatigue, dulled sensorium, lethargy, seizures, coma
SAIDH case study:
58-yr-old female admitted 24 hours ago with small cell CA, hyponatremia, HA, confusion. Na+ 115 on admission. Nursing report this am: confused as to time and place, slight HA last night, A&O x3, Na+ ordered for this am, IV NS 50 mL/hr, fluid restriction 1,500 mL/day
- What will you assess this am? Why will you assess it? Interpret the meanings of the findings. What will you assess about pt/family knowledge?
-What - Orientation x3, HA, GCS, pupil size/reaction, strength in hands and feet, I&O, ask about thirst
- Why - These assess neurological functioning
- Interpretation - Lung CA can produce ADH, very low Na+ may result in brain swelling
- Educate pt and family about fall hazards, tx is restriction of fluids, monitoring thirst, pt may suck on hard candy (sugar- free)
