Prototype: nitroglycerin

Desired effect: Treatment of angina--when O2 req. of heart is greater than the the supply it is getting, the muscle becomes ischemic and causes chest pain

Pharmacodynamics--acts directly on smooth muscle of blood vessels causing relaxation and dilation. HR increases. BP. decreases.

Nursing implications and eval:
1) contraindicated for patients with hypertrophic cardiomyopathy, head trauma, or increased intracranial pressure.
2) Administration: patient sit or lie down, take vitals before and after admin, if becomes hypotensive reposition to facilitate venous return and recheck BP, if remains hypotensive remove ointment or slow IV infusion rate and notify physician, monitor every 5-15 min
3) remove old patches before new, place in different location never over heart, remove before electrical cardioversion
