What is Tetralogy of Fallot?
Pulmonary Stenosis, Overriding Aorta, VSD, right venticular hypertrophy
What happens to blood in the right atrium in Tetralogy of Fallot?
goes to right ventricle, because of pulmonary stenosis gets shunted through VSD into left heart without oxygenation.
How does Tetralogy of Fallot present?
with the murmur of pulmonary stenosis and cyanosis
Why is severe cyanosis from TOF rare immediately after birth?
The PDA allows additional pulmonary blood flow
What is a Tetralogy spell?
sudden drop in arterial oxygen saturation resulting from a sudden increased constriction of the outflow tract to the lungs so that pulmonary blood flow is further restricted.
What happens to a child with TOF when given supplemental oxygen?
Has little effect
What is "pink" tetralogy of Fallot?
normal oxygen levels because pulmonary stenosis is mild
How do you treat a child with TOF whose oxygen level is critically low after birth?
prostaglandin infusion is to keep the ductus arteriosus open and provide additional pulmonary blood flow
When is TOF repaired?
Complete repair is usually done electively when the children are about six months of age, as long as the oxygen levels remain adequate
how is TOF repaired?
Closure of VSD with synthetic Dacron patch, pulmonary valve and right ventricular outflow tract is then augmented by resecting obstructive muscle tissue in the RV
How is long term function after TOF repair?
excellent
What abnormality is left after TOF repair?
leaky (insufficient) pulmonary valve; may lead to RV dilatation
How do you treat a tetralogy spell?
sometimes by comforting the infant and flexing the knees forward and upward. Most often, however, immediate medical attention is necessary.
