Atropine
TX for: Bradycardia
Dose:0.5-1.0mg IVP slowly-- May be repeated every 5 minutes up to 2.0mg total.
S/E: Drowsiness, Urinary retention/urgency, Dry mouth
Adenosine
TX for: Tachycardia
Admin undiluted IV 6 mg rapid IVP...In1-2 mins may repeat with 12 MG
Give over 1-2 seconds....May be followed by Rapid saline flush
May cause aystole or heart block
Other S/S: Flushing, dizziness, CP, and Palpitations.
Watch with pt with asthma may cause bronchospasm
Metoprolol(Lopressor)
TX for tachycardia
Beta Blocker
If HR <50 dont give - hold.
If IV push need to admin slowly and monitor b/p. Dose: 50-100mg
S/E: Bradycardia, Low BP, Fatigue Weakness
Coumadin(Warfarin)
Prevents thrombus in Atrial flutter
Inderal (Propanolol)
TX for atrial flutter
80-320mg/day in divided doses
Look for fluid overload & s/o toxicity (blue palms & nails, bradycardia, fainting)
Cardizem (Diltazem)
TX for atrial flutter
PO: 60-120mg/day in divided doses
IV: .25mg/kg then drip of 10mg/hr
Need ICU or telemetry monitoring
S/E: Hyperplasia of gums, photosensitivity, and Heart block
Calan(Verapmil)
TX for Atrial flutter
IV: 5-10mg slow IVP(May dilute in NS) can cause severe hypotension
PO: 80-120mg/day in divided doses
S/E: Photosensitivity, bradycardia, hypotension. Need to lay recumbent for 1 hr after IV dose.
Amiodarone
Atrial Fib TX
Slows hr down, increases PR and QT Intervals
Dose: 800-1600mg/day initially then 400mg/day maintenance. IV 140mg over 10 minutes, then continuous drip
S/E: Bradycardia, N/V, blue discoloration of skin, numbness in toes and fingers.
Digoxin
Atrial Fib TX
PO: 0.125-.5mg everyday...Take apical pule before hold if &lt;60.
IV: SLOW IVP over min of 5-7 mins monitor pulse
S/E: N/V, Anorexia, Visual changes, bradycardia and heart blocks
Levels need to be .5-2mg/ml
Never give if hyperkalemia or renal failure
Coumadin
If pt has been in atrial fib for more than 48 hrs give this for 3-4 weeks before cardioversion
TEE is done to r/o emboli
INR should be 2-3
Lidocaine
PVC TX
Suppresses conduction
Dose: 50-100mg IVP then start drip (2grams/500CC NS or D5W)
S/E: Drowsiness, Confusion, Hypotension
