Erythematous papules + plaques w/ silvery scaling. Nonpruritic, extensor surf. Acanthosis + *parakeratosis* + Munro abceses. A/w psoriatic arthritis.
Pemphigus vulgaris
Intraepidermal bullae oral => skin. Rupture => 2° infection. IgG vs desmoglein => acantholysis. Potentially fatal.
Bullous pemphigoid
Supepidermal bullae w/ eos. IgG vs BM. Less severe than pemphigus vulgaris
Dermatitis herpetiformis
Purirtic vesicles. Extensor surf. Microabscesses w/ PMN, eos at tips of dermal papillae. IgA at tips (vs. BM). A/w gluten-sensitive enteropathy. Responds to gluten-free diet.
Erythema multiforme
multiple types of lesions. Hypersensitivity to infection, drug, dz. Target lesion: central necrosis surr by lighter erythema
Ocular albinism
Dec melanin
Oculocutaneous albinism
Eyes, skin, hair. Inc risk: actinic keratosis, basal + SCC, malig melanoma. Turosinase-neg or pos.
Vitiligo
Acquired, autoimmune. Anti-melanocyte abs or descruction by other factors
Ephelis
freckle. inc melanin pigment in basal keratinocytes.
Lentigo
Benign melanocytic hyperplasia.
Nevocellular nevus
Common mole. Derived from melanocytes. Junctional, Compound, or Intradermal (often non pig)
Blue nevus
From birth. Highly pigmented melanocytes in dermis.
Spitz nevus
Benign. Spindle shaped cells. "juvenile melanoma" but not.
Dysplastic nevus
Disorderly prolif of melanocytes + dermal fibrosis + lympho infiltration. tform => Malignant Melanoma. AD Dysplastic Nevus Syndrome.
Lentigo maligna
Hutchinson freckle. irreg macular pig lesion on sun-exposed skin. Atypical melanocytes. Tform => Lentigo Maligna Melanoma
Molluscum contagiosum
Umbilicated, dome-shaped papules. DNA poxvirus. Contact
Verruca vulgaris
Benign papilloma by HPV (NOT 16, 18, 31, 33). Koilocytes.
Acrochordon
Skin tag. CT covered w/ stratified squamous
Epidermal inclusion cyst
Surr by stratified squamous, filled with keratinous material. Dome-shaped nodule. "Sebaceous cyst" but not.
Dermatofibroma
Benign. Firm nodule. +/- pigmented acanthosis. "Fibrous histoma" if histiocytosis. Intertwining bundles of collagen/fibroblasts.
Dermatofibrosarcoma protuberans
Slow gorwing, well-differentiated, rarely mets
Keratoacanthoma
Benign. ~ SCC. Regresses spontaneously
Actinic keratosis
Premalignant => SCC. Sunlight => rough plaques.
Acanthosis nigricans
Acanthosis + hyperpig. Marker of visceral malignancy or insulin R
Xanthoma
Yellow papules of lipid-laden histiocytes. Eyelids, tendons, joints. Hypercholesterolemia.
Port-wine stain
Purple-red hemangioma. A/w Sturge-Weber
Strawberry hemangioma
bright-red. > cherry?
Cherry hemangioma
dome-shaped red papule
Cavernous hemangioma
large, endothelal-lined in dermis and subdermis
Sturge-Weber syn
port-wine stain on face, ipsilateral glaucoma, hemangiomas in meninges. Convulsions, MRet, retinal detachment.
von Hippel-Lindau dz
multiple vascular tumors + other organ sys
Granuloma pyogenicum
pedunculated lesion. post-trauma
Keloid
Tumor-like scar from abnormal CT proliferation.
SCC
Scaling, indurated, ulcerated nodule. Locally invasive, infreq met. Sunlight. Lower part of face (vs BCC). Precursor = actinic keratosis. Histo: Sheets/islands neoplastic cells, keratin pearls.
Basal Cell Carcinoma
Most common malig skin tumor. Pearly papule w/ overlying telangiectastia. Locally invasive, rare met. Sunlight. Upper part of face (vs SCC). Histo: dark basaloid cells w/ palisade arr of nuc.
Malignant melanoma
Melanocytes or nevus cells. Sunlight. Radial (no met, in epi + papillary dermis) => vertical growth (reticular dermis, etc).
Lentigo maligna melanoma
Sunlight. From Lentigo Maligna (Hutchinson freckle)
Superficial spreading melanoma
Most common melanoma. Irreg border, variegated pigmentation.
Nodular melanoma
Begins with vertical growth => worst prognosis.
Acral-lentiginous melanoma
Hands + feet of dark-skinned people.
