Front 1

Origin: external occipital protuberance and medial third of superior nuchal line of occipital bone, ligamentum nuchae, spinous processes of the 7th cervical and all 12 thoracic vertebrae
Insertion: Superior fibers into posterior border of lateral third of the clavicle, Middle fibers into medial margin of the acromion and posterior border of scapular spine, Inferior fiers converge to end in an aponeurosis inserted into scapular spine.

Origin: aponeurosis of posterior layer of thoracolumbar fascia, spinous processes of the lower 6 thoracic vertebrae
Insertion: fibers converge into a tendon that inserts into intertubercular groove of the humerus

Latissimus dorsi

Origin: transverse processes of the first 4 cervical vertebrae
Insertion: superior portion of the medial (vertebral) border of the scapulae

Levator scapulae

Origin: lateral third of the clavicle, superior surface of acromion and scapular spine
Insertion: fibers converge into a tendon that inserts into deltoid tuberosity

Insertion: fibers converge, form a tendon, inserts into middle facet of greater tubercle of humerus

Origin: medial 2/3s of infraspinous fossa of the scapula
Insertion: fibers converge form a tendon and inserts into middle facet of greater tubercle of humerus

Infraspinatus

Insertion: medial lip of intertubercular groover of the humerus

Teres Major

Origin: lateral border of the scapula
Insertion: inferior facet on greater tubercle of the humerus

Origin: medial 2/3s of the subscapular fossa and lower 2/3s of lateral border of scapula
Insertion: fibers converge to a tendon that inserts into the lesser tubercle of the humerus and the anterior portion of shoulder joint capsule

Insertion: fibers converge into a flat tendon that inserts into the intertubercular groove of the humerus

Insertion: pisiform bone, an extension to hook of hamate and base of 5th metacarpal. Some fibers also attach to flexor retinaculum
