How do you add an effect in to a clip in the time line on top of an existing effect?

Atl+Drag windows or Option drag mac the effect on top of the existing effect.

The video monitor follows you into the nest so you are able to see what is happening at a given level of the nest without seeing what is happening effects above that level.

You always see the composited result of all the effects in the nest regardless of where you are in the nest; you can hear the audio when you play;you can access your material before and after your effects nest.

Click and drag the effect icon from the effect editor for the effect you wish to reorder.
