Atropine Sulfate
Classification;
*Parasympathetic blocker
*Vagolytic (parasympatholytic)
*Anticholinergic
*Sympathomimetic
Atropine Sulfate
Therapeutic Effects:
*Inhibits parasympathetic stimulation by blocking acethycholine receptors in target organs
*Decreases vagal tone resulting in increased heart rate, SA automaticity, and AV conduction
*Allows brochioles to dilate and decresses respiratory tract secreations.
Atropine Sulfate
Indications:
*Hemodynamically unstable bradycardia, when pacing is not immediately available
*Asystole
*PEA with rate< 60/minute
*Pesticide posioning (organophosphates and carbamates)
Atropine Sulfate
Contraindications:
Know hypersensitivity to the Drug
Atropine Sulfate
Side Effects
(Cardiovascular)
tachycardia, increased O2 demand, bradycardia, increased Mobitz II and 3rd degree block
Atropine Sulfate
Side Effects
(Neurological)
pupillary dilation, blurred vision, dizziness, confusion, restlessness, drowsiness, headache, photophobia, seizures
Atropine Sulfate
Side Effects
(Respiratory)
dry mucus plugs
Atropine Sulfate
Side Effects
(Gastrontestinal)
dry mouth, thirst, difficulty swallowing
Atropine Sulfate
Side Effects
(other)
hot dry skin, worsens glaucoma
Atropine Sulfate
Administration
(Bradysrhythmias)
IVP: 0.5mg rapid,while waiting for pace maker, q 3-5 min to mak of 3mg or 0.4 mg/kg
ET: 2-2.5 time IVP dose diluted with NSS to a minimum vol of 10ml, q 3-5 min to a max of 0.04 mg/kg
Atropine Sulfate
Administration
(Asystole & PEA,60/min)
IVP: 1mg rapid q 3-5 mins to a max of 3mg or 0.04mg/kg
ET: same as for Bradysrhythmias
Atropine Sulfate
Administration
(pesticide Poisoning)
IVP&IM: 2mg q 5-15 mins until patient is asymptomatic. No max dose
ET: 4mg diluted with NSS to a min vol of 10ml q 5 min until sign of therapeutic atropinization . No max dose
Atropine Sulfate
Signs of proper atropinization:
*tidal vol or BVM compliance improves
*SOB & wheezing decreases
*SPO2 and B/P improves
*LOC may improve
*skin and visible secretions dry
Atropine Sulfate
Pneumonic for signs of excessive Atropine administration:
*Red as a beet
*Dry as a bone
*Hot as a Pistol
*Blind as a Bat
*Mad as a Hatter
