If unstable + evidence of tension pneumo ➡ consider decompression |

Suspect tension pneumothorax. Consider for pt w/ PEA as possible revers cause. |

How does tension pneumothorax present? |
At least one of: -Severe resp distress in conscious/breathing pt w/ hemodynamic compromise. -Difficult to ventilate in conscious/apneic pt w/ et tube placed & hypotension. *Also trach deviation |

How to perform pleural decompression? |
-Maintain airway/ventil o2 -IV cath 14g⬆, 2" ⬆ long -Antiseptic wipe, dressing, tape -ID landmarks: insert mid~clavicular 2nd intercostal space, top of 3rd -Prep area, remove flash chamber, insert until air rushes out, advance cath, remove needle. -Reassess breath sounds & pt condition. PEDS 18-20g |
