What are some indicators of high risk pregnancy?
-Maternal age less than 16 or over 35
-Preeclampsia
-Rh isoimmunization
-History of preterm labor
If a mom is hypoglycemic will the baby be also?
Yes, baby will be even more hypoglycemic
Types of Ultrasounds:
-Transabdominal
-Endovaginal (can use this to get closer look at cervix)
What's the most important measurement taken from an ultrasound?
Biparietal diameter of fetal head
If Dr. was going to do an amniocentesis what should accompany it?
Ultrasound
What are doppler blood flow studies used for?
-Evaluate blood flow in the fetus & mother
-Assess placental function
-Helpful in managing pregnancies with maternal diabetes, IUGR, preterm labor, prolonged pregnancies, & multiple gestation
What are Nonstress Tests used for?
To evaluate fetal heartrate with fetal activity
What does a Reactive NST look like?
2 or more FHR accelerations of at least 15 bpm with a duration of at least 15 seconds in a 20 minute interval
What does a Nonreactive NST look like?
Reactive criteria not met within 30 minutes
When is a Biophysical Profile completed? What does it assess?
-When strip is Non-reactive
-Assessment of 5 biophysical variables
1. fetal breathing movement
2. fetal movement of body or limbs
3. fetal tone (extension/flexion of extremities)
4. amniotic fluid vol.
5. reactive NST with activity
-Scoring: 2 is normal
0 is abnormal
Why is a Contraction Stress Test completed?
Evaluates the respiratory function of the placenta
-it records FHR response to stress of uterine contractions
With each contraction the blood supply to the baby is cut off. What oxygenates baby while uterus is contracting?
Placenta
For the CST, how are contractions induced?
-stimulate the nipples (releases Oxytocin)
-Oxytocin IV
Interpretation of CST
Negative:

Positive:
Negative: (good) 3 good contractions lasting 40 seconds in 10 minute interval with no late decelerations

Positive: (bad) persistent late decelerations with more than 50% of the contractions
What's a potential complication with a CST?
-could cause labor- must have permit signed & must be performed in the hospital
What can we tell from an amniocesis?
Triple test - AFP, HCG, & UE3 (UE3/not important to us)
L/S ratio - tells us lung maturity. 2:1=mature
Fetal maturity index
