What is the first line therapy for GAD? Why?
SSRIs. Nearly as effective as benzodiazepines without the side effects.
What is the mode of action of Buspirone?
Partial 5HT1A agontist
What is the advantage of using Buspirone?
Effective as monotherapy, faster onset than SSRI, SNRI ADs. It is well tolerated with no abuse potential. Few side effects
What is one distinct advantage of BZDs over ADs?
Effective in acute panic attack due to rapid onset.
What are the disadvantages of BZDs?
High potential for abuse, potentially fatal with other CNS depressants (alcohol), adverse effects (sedation, ataxia, amnesia, etc)
What would you use to treat GAD with panic? What would you not want to treat it with and why?
BZDs. ADs, because they will actually increase the anxiety symptoms for the first couple of weeks.
What do BZDs do to sleep stages?
Decrease REM and SWS, increase stage 2 NREM sleep
