What is Hypertension?
When the systolic exceeds 140mmhg and diastolic exceeds 90mmhg
What are the therapeutic objectives for Hypertension?
Prevention of organ damage
Reduction of mortality Easiest to do this by lowering BP rather than looking at individual cases |
What can the patient do prior to drug treatment to attempt to lower BP?
Lower body weight
reduce Nacl |
What are the drugs of first choice in hypertension?
Diuretics
B-Blockers |
What is the first option of drug in HT?
Thiazide Diuretics
What if the first option doesn't work, what can you add?
B-Blocker or ACE Inhibitor, AT1 Antagonist or Ca2+ Blocker (Dependant on patient issues)
How do ACE inhibitors assist in lowering BP?
Reduce ANGII = reduced peripheral vascular resistance and reduced BP.
May induce Dry cough, Impair renal function or hyperkalemia (then At1 antagonist) |
Who should not have B-Blockers
Those with lung disease, COPD as can impair pulmonary function
Why are Ca2+ Blockers good and is there a risk?
Dihydropyridines - Long Half Life - beneficial as short Half life drugs rapidly lower BP and elicit reflex Tachy.
Is mono therapy the best or combined therapy?
Not really combined is better.
