Atropine Sulfate
-Parasympatholytic drug used to treat Bradycardias
-Anticholinergic
-Side effects- Blurred vision Dilated pupils, drymouth, tachycardia, and drowsiness.
IV/IO .5mg every 3-5 min till 3mg
Lidocain
-for stable monomorphic ventricular tachycardia.
-Weak sodium channel blocker
-Initial dose: 1-1.5 mg/kg IV/IO, repeat if needed every 5-10 minutes @ .5-.75 mg/kg IV/IO till 3.0 mg/kg
Side effects:
Adenosine
Manage Narrow complex tachycardia(stable/unstable)
Side Effects: burning sensation, heaviness in arms, hypotension, Chest pressure, diaphoresis, numbness, tingling, dyspnea, headache, nausea, vomiting
contraindications: 1st,2nd blocks and asthma IV
Initial dose 6mg w/ 20ml flush (can do 12-12) till 30mg
Amiodarone
-Cardiac arrest w/ shockable rythm unresponsive to cpr, d-fib and vasopressor therapy
-stable narrow-complex tachycardia (a-fib)
control of rapid ventricular rate
contraindication: cardiogenic shock, bradycardia, high degree heartblocks
-300mg IV/IO-150mg IV/IO- 150mg IV/IO
Diltiazem
-Calcium channel blocker slowing heart rate in SVT
-Stable narrow complex tachycardia if untreated by vagal maneuvers or adenosin
-A-fib/ Aflutter
-15-20mgIV over 20 min (repeat dose over 15 if needed)
