LD:
Classify
Interference
-Oxidoreductase
-gross hemolysis significantly elevates – RBCs contain aLOTTA lactate dehydrog.
-unstable, store below 25.
Main use: elevated 10 days, takes too long for diagnosis on this basis (72-144 hrs for peak, normal for first 12 hrs).
CK:
Classify
Interference
-transferase
-no CK in RBCs – HEMOLYSIS NOT AS BAD.
-AK IS in RBCs, it reacts w/ ADP -> ATP, falsely elevates assay.
-Daylite falsely decreases levels.
-High amounts of CK = falsely elevated, poor sensitivity.
Main use: MI because peak is within 24 hrs; begins to elevate within 7-18 hrs, much sooner than LD. Still look at LD because CK excreted by kidneys within 2-3 days.
AST
Classify
Interference
Transferase
Hemolysis increases by 10x
ALT
Transferase
Hemolysis increases levels
ALP
Hydrolase
pH 9-10, Mg2+
Anticoags can't bind Mg
Hemolysis increases 6x
5'Nucleotidase
Hydrolase
GGT
Transferase
None in RBCs, no effect by hemolysis.
AML
Hydrolase
Run serum and urine cuz it's excreted.
LIP
Hydrolase
Filtered, but not excreted.
Hgb inhibits lipase, avoid hemolysis.
ACP
Hydrolase
Acid conditions
hemolysis will falsely elevate.
Aldolase
lyase
hemolysis will falsely elevate
Cholecystikinase
Hydrolase
RBCs have AChE, mimic this, falsely elevate.
