What are the 4 types of hypersensitivity reactions?

Type I: immediate; IgE mediated. Have an acute phase, and then late phase


Type II: IgM or IgG Ab bind to fixed tissue antigen (ie BM like Goodpasture, ACh receptor like M gravis


Type III: Immune complex. Think vasculitis (serum sickness, arthus reaction)


Type IV: delayed type (24-72 hr)


Type V: Autoimmune (IgG and stimulatory effect. Think Graves

What is the difference between anaphlyaxis and anaphylactoid?

Anaphylactoid - not IgE mediated. Ie Aspirin, NSAIDs, contrast




You cannot clinically distinguish between the two.

What is difference between acute and chronic hives?

Duration ( 6 weeks)


For chronic majority is idiopathic.


When find a cause, often it is autoimmunity

Urticarial Vasculitis

When Lesions last > 24 hours in a FIXED location


- residual ecchymosis, hyperpigmentation




Dx: skin biopsy

Allergic Rhinitis

Seasonal (grass, tree, weed pollens)


Perennial (dust mites, molds, dander)


- nasal congestion, rhinorrhea, sneezing, nasal pruritis


- IgE mediated


- nasal smear shows eosinophilia


PE: Dennie Morgan lines, allergic shiners, "nasal solute"


Rx: avoidance, antihistamines, allergen specific immunotherapy

Tree pollen


Grass Pollen


Weeds

Tree pollen - spring


Grass pollen - early summer


Weeds - Fall

"NARES"

Non Allergic Rhinitis with Eosinophilia syndrome


-- chronic nasal congestion


-- do NOT demonstrate sensitization to any allergens

Allergy Immunotherapy

Only works for IgE mediated conditions


-- give increasing dose of allergens via SQ to induce immune tolerance


-- also get production of blocking antibodies IgG isotype to IgE
