How is the LMA placed in the hypopharynx?

index finger caudally guides cuff along hard palate and down into hypopharynx until increased resistance is felt

GERD, PPV, duration of use, position, surgical procedures, pathology of mouth, pharynx or larynx, restricted access to the airway
