Cardiac Tamponade
Decreased CO
Decrease Contractility Decrease Perfusion Muffled Heart Sounds JVD Pulsus Paradoxus Restlessness Upright, leaning foward posture CXR = Cardiomegaly TX = Pericardiocentesis Meds = Dopamine/Epinephrine |
Pulsus Paradoxus is a decrease in systolic BP with inspiration
CHF or Left Sided Heart Failure
Decreased CO
Pulmonary symptoms (SOB, Dyspnea, moist cough, crackles and S3+S4 heart sounds) Elevated BNP levels ABG's = hypoxia/hypercapnia CXR = congestion/L ventricular hypertrophy TX = O2, decreased sodium & fluid intake Meds = ACE inhibitors, Digoxin, Diuretics and Nitroglycerine |
ARDS or Acute Respiratory Distress Syndrome
Fluid in lungs = stiffness
Decreased O2 & perfusion Altered LOC Anxiety/Restlessness Adventitious breath sounds Dyspnea ABG's = metabolic/respiratory acidosis Hypoxia that doesn't respond to F102 Culture = infection CXR = infiltrates TX = bed rest, passive ROM, physiotherapy, suction, restrict fluids, O2 - peep. Meds = Bronchodilators, A/B, and steroids. Monitor I/O's. Reposition q2h, high fowlers position, TPN or enteral feedings, and daily weight |
Pleural Effusion
excess fluid in pleural space
Decrease breath sounds Dyspnea Fever Pleuritic chest pain CXR = fluid Thoracentesis = lactate dehydrogenase (LD) levels less than 200 IU and protein less than 3g/dL. TX = thoracentesis w/ chest tube if needed Admin O2 and monitor for complications of thoracentesis Chest tube care and drainage monitoring |
