513: ANESTHESIA MACHINE
513: ANESTHESIA MACHINE
Functions of the anesthesia:
1. receives gases from central supply system.
2. control flow of desired gases
and reduce their pressure.
3. measure amount of gases and load them with anesthetic vapors.
4. provide gas to pt
5. provide monitoring and safety features.
Datex-Ohmeda
1. excel
2. aestiva
3. asiys
4. avance
Drager medical
1. Apollo
2. Fabius GS
Identify high pressure machine components.
1. hanger yoke
2. yoke block
3. cylinder pressure gauge
4. cylinder pressure regulators
Identify intermediate pressure machine components.
1. pipeline inlets & pressure gauges.
2. Gas power outlet
3. Master switch (pneumatic component)
4. O2 flush
5. O2 pressure failure device (fail safe) & alarm.
6. O2 & nitrous oxide second stage pressure regulators.
7. flowmeter valves
Differentiate between ON & Standby modes.
Standby (O): machine is not really OFF b/c there are portions of the machine that are still active (i.e. O2 flush, gas power outlet, electrical outlets, and battery recharging circuitry).
Explain how O2 flush valve works & possible hazards associated with its use.
Receives O2 from pipeline inlet or cylinder pressure regulator & directs a high metered flow (35-37 L/min) to common gas outlet.

Can be accessed whether the master switch is turned ON or OFF.
Explain how O2 fail-safe device funcitons in general & how it works in Datex and Drager machines.
Datex machines stop the flow of nitrous oxide and air once O2 pressure < 20 psi.

If O2 pressure is normal, plunger and seal assembly are depressed to allow anethetic gas flow through the valve.

The valve closes when O2 supply pressure fails completely.
Describe relationship between the fail safe device & the alarm in the Excel machine.
.
State the functions of secondary pressure regulators including gases used, final pressures, and purpose.
1. reduce intermediate pressure to low pressure (20 psi for O2 and 38 psi for N2O)

2. decrease pressure fluctuations.
Compare and contrast the different gas control knobs.
O2 control valve:
1. has fluted(pegs around edeg) profile.
2. as large or larger than other gases.

All other gas knobs: MUST BE ROUND.

NOTE: the O2 control knob can be felt distinctly due to its pegged (fluted) shape.

Counterclockwise to increase flow.
Identify low pressure machine components.
1. hypoxic guard/ proportioning system.
2. flowmeter tubes
3. vaporizers
4. check valves if present
5. common gas outlet
Compare and contrast the design of the Link-25 to the O2 Ratio Monitor Controller.
O2 Ratio Monitor Controller is a hypoxic guard system.
Illustrate the sequence of flowmeters & explain the reasoning for the design.
Flowmeter sequence can be a cause of hypoxia.

If there is a leak downstream of O2, it will be lost creating a hypoxic mix.

Anesthesia machine requires O2 to be the LAST (right most gas) gas before the manifold outlet because it is the safest arrangement. This way leaks may occur upstream, and O2 can still be running.
O2 fail safe device/alarm in Drager?
Nitrous flow decrease porportionally with O2 pressure. to maintain FiO2 > 19%.
When does the O2 fail safe alarm sound?
When O2 pressure is < 30 psig.
Link 25 Proportioning system
It's a hypoxic guard system.

Minimize the risk of delivering a hypoxic mixture to pt. by maintaining a minimum of O2 conc. 25% in fresh gas supply, hence the name Link 25.
Explain the function of the "Link."
Helps to ensure that when the nitrous oxide flow is more than 3x the O2 flow, the O2 flow control valve opens to provide at least 25% of the overall flow mixture.
