CV change in pregnancy:
- blood volume increase by 45%
- CO increase 1-1.5L/min
- resting heart rate increase 15-20 bpm
- BP fall by 10-15 in 2nd trimester and raise again in 3rd
- Twave inversion or flat T in II, V1, V2 and Q in II aVF
- LAD
- IVC obstruction
Haematological changes:
- Dilutional anaemia
- induce leukocytosis
- increase ESR
- Coagulaion faction increase --> venous thrombosis
Resp changes:
- increase Tv by 40%
- elevated diaphram, decrease residural volume by 25%
- resp alkalosis
- hypocapnia pCO2 30mmHG
- wide mediastinum on CXR
Other changes:
- delay gastro emptying, aspiration risk
- dilated renal pelvic, hyperaemic bladder , more susceptible to trama
- increase uterine size, shielding bowel, more susceptible
Specific injury ass with pregnancy
- pelvic fracture, difficult to diagnose retroperitoneal haemorrhage
- placental abruption
- uterine rupture
- fetomaternal haemorrhage , rhesus sensitization (Kleihauer Betke test)
