Assesses client’s previous or perceived experience with IV therapy and arm placement preference; fluid or electrolyte imbalances; assesses laboratory data and client’s history of allergies; assesses for the following risk factors: child or older adult, presence of heart failure or renal failure, or low platelet count; determines if client is to undergo any planned surgeries or is to receive blood infusion later..

3. Prepares client & family by explaining the procedure, its purpose, and what is expected of client. Assists client to comfortable sitting or supine position.

5. Organizes equipment on clean, clutter-free bedside stand or over-bed table. Changes client’s gown to the more easily removed green gown with snaps at the shoulder, if available.

6. Opens sterile packages using sterile aseptic technique.

7. Checks IV solution using five rights of drug administration. Makes sure prescribed additives have been added. Checks solution for color, clarity & expiration date. Checks bag for leaks. This is best if done before reaching the bedside.

8. Opens infusion set, maintaining sterility of both ends of tubing.

9. Places roller clamp about 2 to 5 cm (1 to 2 in) below drip chamber & moves roller clamp to “off” position.

10. Removes protective sheath over IV tubing port on plastic IV solution bag.

11. Inserts infusion set into fluid bag. Removes protector cap from tubing insertion spike (keeping spike sterile) & inserts spike into opening of IV bag.

12. Primes infusion tubing by filling with IV solution. Compresses drip chamber & releases, allowing it to fill one-third to one-half full.

13. Removes tubing protector cap (some tubing can be primed without removal) & slowly releases roller clamp to allow fluid to travel from drip chamber through tubing to needle adapter. Returns roller clamp to “off” position after tubing is primed (filled with IV fluid).

14. Checks that tubing is clear of air & air bubbles. Replaces tubing cap protector on end of tubing.
