1. Verifies physician’s order.

2. Assesses client’s history of allergies, responses towards receiving injections, contraindications.

3. Prepares correct medication dose from ampule or vial.

4. Identifies client; checks identification bracelet with MAR & asks client’s name.

5. Explains steps of procedure & tells client injection will cause a slight burning or sting; closes door or pulls room curtain; washes hands; applies disposable gloves; drapes patient appropriately.

6. Selects appropriate injection site. Inspects skin surface over site for bruises, inflammation or edema. Notes integrity and size of muscle and palpate for tenderness or hardness. Avoids these areas. If injections are given frequently, rotates sites.

7. Has patient lie flat, on side, or prone, depending on site chosen. Relocates site using anatomical landmarks.

8. Cleanses site with an antiseptic swab, applying swab at centre & rotating outward in a circular fashion for about 5 cm (2 in)

9. Holds swab between third or fourth fingers of non-dominant hand.

10. Removes needle cap by pulling it straight off. Holds syringe between thumb and forefinger of dominant hand

11. Administers injection:

a) Positions non-dominant hand at proper anatomical landmarks & pulls skin down to administer in a Z-track.

b) If client’s muscle mass is small, grasps body of muscle between thumb & fingers.

c) Inserts needle quickly at 90 degree angle into muscle. Aspirates to check for blood. If blood appears in syringe, removes needle & disposes of medication & syringe. Repeats preparation procedure.

d) Injects medication slowly

e) Waits 10 seconds. Smoothly & steadily withdraws needle & releases skin.

12. Withdraws needle while applying alcohol swab gently over site.

13. Assists client to comfortable position. Discards equipment into appropriate receptacle. A one-handed technique can be used to recap a needle. Removes disposable gloves & wash hands. Stays with client 3 minutes to observe for any allergic reactions. Charts in MAR & reports any undesirable side effects

14. Returns to evaluate client’s response to medication in 10-30 minutes.
