What causes a recurrent subareolar abscess to develop from periductal mastitis?
When the squamous metaplasia produces much keratin which creates a keratin plug
What type of inflammatory response develops in response to the keratin plug?
Chronic granulomatous inflammation
What is a Mammary Duct Ectasia?
A Poorly defined periareolar mass with thick nipple secretions that can even be blue or green
In what patients is mammary duct ectasia more likely to be seen?
50-60 yr olds that have given birth to multiple infants.
What type of morphologic changes are seen in mammary duct ectasia?
-Duct dilation
-Lipid laden macrophages in duct
-Periductal chronic inflammation and fibrosis
What is RARELY seen in the periductal chronic inflammation of mammary duct ectasia?
Granulomas
What does Fat necrosis in the breast often follow?
Trauma or prior surgery
How does Fat necrosis generally present clinically?
As a painless palpable mass that mimics carcinoma because of skin thickening or retraction
Why is fat necrosis often a cause of biopsy?
Because when mammogram is done to investigate the palpable mass it often shows density or calcifications
What is another thing that can cause calcification due to chronic inflammation?
Reaction to silicone implants
What type of inflammatory response will likely be seen in a silicone implant reaction?
Granulomatous
