No! You don't want to cause congestion under the cast.

2 weeks

Is light massage a good way to bring blood to a stress fractured site?

No! On-site massage is CI as the fracture site is point tender.

What should you watch out for after the client takes off the cast?

Tissue could be fragile and sensation is altered. No extreme hydro temperature and no deep longitudinal techniques.

Because you may compress over carotid artery or carotid sinus and client may feel "choked".

Yes! Caused by compression of fourth sacral nerve root S4. (green book page 629)

For a client with hemiplegia, what do you need to watch out for when working on the neck?

1. SCM light pressure, only one at time.


2. Don't extend and rotate the neck at the same time as that may occlude vertebral artery


3. Sensory may be deficit.

Because MS clients suffer from spasticity, it is a good idea to perform deep techniques to reduce spasm?

No! Deep and aggressive techniques can increase sympathetic system firing which will fatigue the client. Do very slow, gentle, stretch.




This applies to Parkinson, Cerebral Palsy

What is Autonomic Dysreflexia and what is it related to?

Related to spinal cord injury (usually above T6) that causes a Sudden onset of Very High Blood Pressure.

List 4 conditions that may happen on a client with spinal cord injury.

1. Heterotopic Ossification (formation of bone in muscle or connective tissue)


2. Deep vein thrombosis


3. Autonomic Dysreflexia


4. Decubitus Ulcers

For a peripheral nerve injury, what is a key thing that tells you it is okay to put hydro or moving the joints of the affected limb?

Return of the muscle function! If muscle function has returned, then you know the nerve lesion has healed.

How long do you need to wait before you can work the affected limb after a crush/severance of peripheral nerve?

At least 3 weeks, no massage at all because the healing of nerve is very slow and delicate. Need to consult with neurologist before you can work "on-site".

Is Deep Moist heat okay in the neck and anterior chest area if he has hypertension or atherosclerosis in the neck vasculature?

No! Also no muscle stripping or aggressive techniques in these circumstances.

Hip and Sacral mobilizations are okay with 3rd trimester of pregnancy?

No! Also very careful with osteoarthritis or degenerative condition

Is Thyme essential oil good with pregnant or hypertension client?

No!

A client with kidney problem wants to thin out the mucus by drinking more water. Is this a good idea?

No! Do not increase water intake if the client has cardiac and renal disorders. IE: Heart failure cant' drink too much water.

Should you continue postural drainage for chronic bronchitis if hemoptysis happens?

No! (blood in sputum)

A client with severe Emphysema should not lie supine and prone right?

Yes.

Should you get consent from the client's physician if client has severe hypertension?

Yes.

Is cupping, hacking and pounding a great way to treat chronic bronchitis on a client with severe hypertension?

No! Too stimulating which increase sympathetic nerve firing = increase blood pressure.




No Stimulating techniques. For trigger point and friction, do it in limited area only and with a lot of gentle and soothing techniques.

To slow down Diarrhea, it is good idea to give a gentle massage to slow down intestinal activity?

No! Diarrhea is CI for abdominal massage.

During flare-up of Inflammatory Bowel Disease, you need to avoid abdominal massage but low back is okay?

False. Avoid BOTH abdominal and low back.

Is it okay to use joint play slowly during acutely inflamed joints?

NO!

Should you give Flexion or Extension exercise to a client with osteoporosis?

Extension because Flexion (like sit-ups) compress on the spinal cord and may cause compression fracture. Avoid bending forward from a standing position

Is extreme hydro indicated for a client with decubitus ulcer?

No because those with decubitus ulcer generally have decreased tissue health (they are not moving much). They may have some sensory loss especially with Diabetes.
