1. What stages of change is Peter currently at? How will this affect your working with Peter?

2. What are Peter’s current social and emotional wellbeing issues?
Peter social and emotional wellbeing issues include:
Isolation – limited contact with family, no longer participating in his church group and no involvement in any social activities. This disengagement with his family and community could be impacting his social wellbeing.
Inconsistency with medication.
Peter is at risk of homelessness and needs to have his accommodation issues addressed as a priority. Not having a stable living environment can be impacting psychological wellbeing, in particular, his environmental mastery.

What changes might Peter like to make in his life?
Peter wants to change his lifestyle and improve his physical health. Peter will need assistance with finding himself some secure accommodation. Then depending on what Peter sees as his priority he may want to work on his alcohol and tobacco consumption and how this impacts his health and finances. He may wish to looking at healthier and affordable meal options available within his budget. Also Peter could look into adding some physical activity into his lifestyle.
4. What barriers might Peter experience in making his chosen changes?
Most significant barrier is Peter’s accommodation. Secure accommodation will need to sort before Peter can begin to address other areas of his life.
Peter smoking and alcohol consumption take up a lot of his money. Without this burden on his finances he may be able to afford healthier meal options. However, giving up or reducing the alcohol and smoking intake can be difficult, especially the alcohol which may need medical supervision.
Peter inconsistency with taking his anti-psychotic medication. Being on the medication may reduce his psychotic episodes, however, Peter also finds it difficult to manage the side effects of taking the …show more content…
It may involve encouraging Peter to be part of his church group again, seek out other community based activities and see if there are any suitable peer support programs available.
6. What approaches would you use to work with Peter to promote his social and emotional wellbeing? Why do you think these approaches would be useful?
Education in healthier food options available to Peter. If available seek referral to dietician. Ask Peter about the last time he seen his doctor and if needed encourage Peter to see his doctor to have a check-up, provide support by attending the GP with Peter (only with Peter’s consent)
Smoke Cessation counselling/support. Provide most update information available on quit smoking.
Provide support throughout the process from beginning until Peter feels he has successful achieved each goal.
Combination of preventing mental illness with the support of his physician and personal recovery methods like positive psychology. Using a collaborative approach from all services that are involved or going to be involved in Peter’s management of his
