Reason for Visit: Unscheduled Visit Right Elbow Pain
S: Right Elbow pain 7/10, spasm like pain, pulsation, along with right hand numbness. This was some time event, now it is a daily occurrence and she is concerned.
O: Old scar tissue on the right elbow, no warmth palpated, no spasm noted, right hand grip strength 4+/5+, Tinel's Sign test was negative, full ROM, NVS intact, radial and ulnar pulse +3; brisk capillary refills
P: Refer back to Dr. Turki.
RESTRICTED DUTY: No repetitive twisting/turning; no vibratory tools with right hand; F/U after ortho.
