CHIEF COMPLAINT: Modified barium swallow study.

Barium swallow study completed 11/15/2017.

Applesauce: Mild residue in the vallecula sinus. Compensatory strategies: Alternating with thin liquids to clear the residue, although it was helpful, he aspirated the thin liquids.
Pudding: Moderate residue in the vallecula sinus. There is also mild to moderate residue in the pharyngeal vestibule between the vallecula sinus and piriform sinus secondary to decrease pharyngeal constriction. Residue in his throat created him to "gag and hack up" the residue back into his mouth. Compensatory strategies: Alternating with thick liquid help to clear but patient demonstrated penetration during the swallow secondary to the residue silently.
Peaches: Mild to moderate residue in the oral cavity. Mild residue in the vallecula sinus. Residue from oral cavity after his first swallow eventually fell into his vallecula sinus and space above the piriform sinus. Again causing patient to gag and regurgitate bolus back into his mouth and in his nose.

Patient has already participated and understands how to complete oral and pharyngeal exercises. He has been in therapy since 07/2017. Patient has been noncompliant completing exercises on a regular basis.
2. SLP will explain the results of the modified barium swallow study and help educate him on compensatory strategies in which he should utilize based on the modified barium swallow study and the SLP will monitor him once a month for 2-3 months. Since patient has not been compliant with exercises, he is not a good candidate for consistent treatment on a regular basis.
3. Diet consistency: Soft mechanical with honey thick liquids. Alternating bites of food with liquid. Swallowing each bolus three times. Patient needs to stop regurgitating/hacking residue to oral cavity as it causes it to also go into his nasal cavity which will possibly result in infection in his
