Strict glycemic control through diet, exercise and pharmacotherapy intervention is essential. Human kidney has great role in maintaining glucose homeostasis. Gluconeogenesis, glucose utilization and glucose reabsorption are the 3 mechanisms that take place in kidney. Reabsorption has the most important role in supporting glycemic state. SGLT2 is found in the proximal tubules of the kidney and intestine, which contribute to renal glucose reabsorption. “In diabetes 2, filtered glucose in the tubules exceeds the transport maximum and gets excreted via urine even though the transporters continue to function normally” (Dixit. etl, 2015). Empagliflozin is a direct inhibitor of SGLT2, which reduces renal reabsorption of filtered glucose. This medicine helps to drop the renal threshold and increase urinary glucose excretion. Inhibiting SGLT2 results increased glycosuria and drop in plasma glucose concentration. The medicine is useful for diabetic patient since it helps to maintain glycemic homeostasis. It does not depend on insulin. Blood sugar can be controlled without having any risk of low blood sugar.
