S.M.A.R.T plan; Specific; Mrs Y and myself were involved in this plan. I would like to provide person centred care. Mrs Y has chosen an activity to make some Christmas decorations while having a cup of tea and conversation. To accomplish this activity we will need the materials for the decorations of which we can find at Saltgreens day centre and Mrs Y and I have arranged to do this on the 6th of December a day that she attends the day centre. Measurable; This goal set does not have any additional cost to Mrs Y. Mrs Y and myself only thus providing person centred care. Mrs Y will be able to tell me if she had a good time or not. We agreed to stay and do this activity at Saltgreens incase Mrs Y needed to attend the toilet as Saltgreens has the equipment necessary to support Mrs Y. Attainable; Mrs Y has chosen to make a “chain decoration”. We have arranged this activity for the morning which should give us enough time to talk about anything Mrs Y
