The core of healthcare is structured to encompass educational, preventive and therapeutic approaches to reduce costs and lead to better therapeutic outcomes. The principle of this healthcare model is built, and is readily adaptable for health professionals, individual patients, targeted groups, or practice populations. By attending Xavier University College of Pharmacy the major task is to prepare pharmacists to impact medically underserved communities, particularly African Americans, in an effort to eliminate health disparities through patient-centered care, community service and scholarly work. As the general community attitudes develop, health care costs spiral, and evidence accumulates, about four billion prescriptions are dispensed every year, and there is estimated to be one mistake for every 67 prescriptions dispensed by a pharmacy every day. This potentially means there are 60 million dispensing errors every year.1,2,3,4,5,6,7 A medication error is defined as "any preventable event that may cause or lead to inappropriate medication use or patient harm while the medication is in the control of the health care professional, patient, or consumer.

This is on top of any confusion caused by difficult handwriting interpretation. Patient safety is put at risk with abbreviations although it saves time initially it later creates more work for other professionals. The prescriber has to be contacted and asked what an abbreviation means, to prevent uncertainty of initial order that puts patient in harm as the result of confusion caused by an abbreviation. In the entire scope of the situation this abbreviations actually end up costing time and other substantial costs. Simply avoiding abbreviations goes a long way toward preventing medication
