By prescribing to the patient, there should be minimizing medication error so that it can be safe for the patient to apply. “Pharmacists bring a specialized perspective that allows for the evaluation of medication processes for safety and for the design and implementation of solutions for improvement that work within existing medication structures.”3 In other words, the pharmacists do not only deal with prescribing medicine but also take charge of the effectiveness of drug in every perspective, which include the medication safety. The study from Havard Medical Practice revealed that not all the medication are beneficial to the patients because many patients became victims of either medication error or adverse drug event. This was the most shocking information because even though I have heard about medication error, I never expected to hear that there still are many people who are suffering from the medicine. It is reasonable that there might be human error during medicine prescription but one should consider this job or duty very seriously since this can unintentionally kill the innocents. As the world is continuously evolving, it is not surprising that variable and mutation occur so that the same medication does work for every time.
