Summary Of A Clinical Reflection

November 12, 2015, 8 am third day of clinical, I was assigned to shadow an RN named Senaite who was working at the 300 hall, where most of the client are not bed ridden but there are some who are not mobile. I was asked to check blood pressure of a client. I introduced myself, washed my hands, checked the client’s name, and explained the process. I asked the client which hand he prefers, the client replied “it does not matter for me,” then I checked the blood pressure, but it was too low or abnormally low for someone like him mentally active and alert. While I was checking his blood pressure, he was listening to the radio on his headphone, and flipping between TV stations, which confirmed to me that he was able to express any pain or abnormality. Finally, I thanked the client, left the room, and explained to the RN my findings of the BP and why it was that low.
It turns out the client is on dialysis, the RN told that the low blood pressure is normal for him. Then I decided to Google why a client on dialysis has a low blood pressure and I learned that dialysis can cause …show more content…
As we approached the hall my instructor explained to us about a situation which required one of the clients on the 300 level to be taken to a hospital. According to the RN that I worked with, the client that I checked the blood pressure the previous day was taken to hospital because he was worried that something was wrong with him, he did not feel normal. I went back again to Google about the relationship between dialysis and low blood pressure, I learned it is very important to check the cause to low blood pressure for clients on dialysis. Typical symptoms of low blood pressure include nausea, sweating, chest tightness, shortness of breath, paleness, difficulty in breathing, restless, blurred vision, etc.
